Patient Medical History Form 

It is necessary for Be Dental to obtain your personal information as well as a detailed history, in order to provide you with the best level of care. Therefore it’s important for our healthcare providers to know the medical history of their patients as it allows them to identify any issues or conditions prior to consultation.

Please complete your personal information as accurately as possible so that we can provide you with the best and safest dental care. Your records are kept confidential and will not be disclosed to any third parties. 

Child Dental Benefit Schedule Bulk Billing Consent Form 

We require a signed consent form as acknowledgement of bulk billed treatment and any fees it may incur.

A medical release form is a document that gives healthcare professionals permission to share patient medical information with other parties.

Cancellation Policy 

We respect that your time is valuable and we appreciate that you understand ours is too.

We have in place a 24 hour cancellation policy, what this means is any cancellations that occur 24 hours or more before the scheduled time are able to be re-booked at no additional cost to the patient. The first time a cancellation is made within the 24 hour period prior to the time of the booked appointment, the patient will be given the opportunity to re-book (with no financial penalty applied). Any subsequent cancellations by/for the same patient that occur within the 24 hour period may result in a cancelled booking fee being applied (which will be equal to the value of the consultation fee for that booking which had been made and subsequently cancelled). 

Patients who make a booking and subsequently fail to present for their designated appointment (and have not contacted the practice to cancel their appointment), may result in a cancelled booking fee being applied. 




Do I need to bring my Health Fund card with me?

Yes, unfortunately we physically need to swipe your health fund card through our HI CAPS machine on the day of treatment.

Can I get a quote from my Health Fund, without needing to call or visit them?

For participating health funds fortunately we are able to swipe your health fund card through our HI CAPS machine and as a result, give you an estimate on your benefit on the spot. Enabling you to make an informed financial decision, before receiving your service.

Do I need to pay the full fee before I can claim my health fund rebate?

There’s no need to pay the full consultation fee up-front then wait for the reimbursement of your benefit. Once your claim is authorised by the health fund, you simply pay the gap amount – the difference between the full fee for the treatment and the amount claimed from your health fund.
